TEAMS AND LEAGUES PLEASE NOTE THE FOLLOWING CHANGE FOR 2010:

Teams and leagues that use Bollinger insurance MUST register their teams individually beginning in 2010.

Forms without complete fee will be returned. All returned checks are subject to a $25.00 fee 

NOTES:

A.    This form must be completed and submitted for teams that wish to be classified as Class B. Teams wanting to be classified as Class B will
need to submit the RegisterASA invoice, the MASS ASA Class B Waiver Forms, and a check to register/insure the team. Once these forms have
been submitted, they will be reviewed and a classification made. The team contact will be notified via e-mail. Please note the new address for
Class B forms only.

1.    This form is a generic entry form. Please visit the appropriate tournament's website for their entry form. Unless otherwise indicated by the tournament, teams may have to play on Friday night.

2.    This form must be completed for each age division hosted. The form will be submitted to the appropriate Area ASA Commissioner for review along with the Deputy State JO Commissioner. Once approved, the tournament will receive via e-mail, a sanction certificate which must be posted at the tournament site. Tournaments not posted on the MAJOASA website nor have this sanction certificate are NOT sanctioned by the MAJOASA.

3.    This form submit be submitted when you register either your entire team or just the coaches individually. The MASS ASA cannot accept just this form
to process the background check. ASA charges a nominal fee to run the background checks. The MASS ASA does not see the actual background
checks. It only receives either a green light to approve the coach or a red light showing the coach has an issue with the background check.
