Forms without complete fee will be returned. All returned checks are
subject to a $25.00 fee
NOTES:
1. This form needs to be complete regardless of
whether the team is individually registering and insuring through
Bollinger insurance or the team
has its own insurance. Teams wishing to be classified as Class B or
Class C must complete all forms in their entirety. Failure to complete
all of the forms will
delay the team in being processed and classified.
2. This form must be completed for each age division
hosted. The form will be submitted to the appropriate Area ASA
Commissioner for review along with the Deputy State JO Commissioner. Once
approved, the tournament will receive via e-mail, a sanction certificate which
must be posted at the tournament site. Tournaments not posted on the
MAJOASA website nor have this sanction certificate are NOT sanctioned by
the MAJOASA.
