NOTES:

1.    ASA has removed the requirement for travel permits starting in 2008. You will need to have your ASA Championship Roster signed to prove you are registered with the MAJOASA.

2.    This form is a generic entry form. Please visit the appropriate tournament's website for their entry form. Unless otherwise indicated by the tournament, teams may have to play on Friday night.

3.    This form must be completed for each age division hosted. The form will be submitted to the appropriate Area ASA Commissioner for review along with the State JO Commissioner. Once approved, the tournament will receive
       via e-mail, a sanction certificate which must be posted at the tournament site. Tournaments not posted on the MAJOASA website nor have this sanction certificate are NOT sanctioned by the MAJOASA.
